Diabetes & Thyroid Membership

Overview:

This membership is designed for uninsured patients or patients whose insurance is not accepted who need regular diabetes or thyroid care. It provides structured medical management with affordable monthly pricing.

Pricing:

● $75 per month for diabetes care or thyroid care.

● $100 per month if you have both conditions.

● Memberships are billed quarterly.

Labs:

Laboratory testing is not included in the membership fee and is paid separately.

1) Diabetes panels: Hemoglobin A1c, lipid panel, complete metabolic profile, urine albumin

2) Thyroid panels: TSH, Free T4, and T3.

Cost: $75 per laboratory panel.

Medications:

Medications are not included in the membership. Medication costs vary depending on the treatment plan and whether standard or compounded medications are used.

Visit Frequency:

Visits occur every 3–6 months depending on disease control and treatment adjustments.

What’s included:

Quarterly physician visits (first visit in person, follow‑ups by tele‑health if appropriate). Laboratory panels cost $75 each and are ordered only when clinically necessary.
